To initiate the "Canker" side quest, you must converse with Gules at Semine. The ideal moment to pick up this quest is during the wedding of Lord Semine and Agnes. Approach Gules while he's enjoying a drink before the ceremony and start the quest. Alternatively, you can trigger this quest earlier by visiting Semine and speaking to Gules during his patrol. Inquire about his past association with bandits and volunteer to assist in eliminating the remaining threats. Canker is the first bandit on your hit list.

Find and Kill Canker

Once you accept the quest, Canker's location will be marked on your map, north of Nebakov Mill. Be cautious as the path to this location is prone to bandit ambushes. You can opt for stealth to avoid them, engage in combat, or simply flee. While these random bandits are not particularly challenging, it's best to conserve your health for the upcoming fight.

Canker's camp is nestled in the mountains, accessible through a narrow gap, similar to the Hermit's house in the "Wedding Crashers" quest. Prepare for battle before entering, as you'll face several bandits alongside Canker. Some bandits may flee depending on the order in which you engage them, but you must eliminate most of them. Fortunately, Canker will not escape, ensuring you can complete the quest by killing him.

Finish the Quest and Return to Gules

After defeating Canker and his crew, loot Canker's body to collect the Light Mace, which serves as proof of your success. Feel free to take any additional items you find valuable. Once you have the mace, return to Gules to conclude the quest.

Upon completing "Canker" in Kingdom Come: Deliverance 2, you'll need to wait a full in-game day before you can proceed with the next quest involving Gules' former bandit associates. After the day has passed, speak with Gules to start "Handsome Charlie."
